Girls Blue Card Registration

STEP 1: APPLY FOR BLUE CARD

To apply for a Blue Card you can either download a Blue Card application form or complete an online application. Please follow the links below:

You will require to provide two forms of identification with your application and your identification needs to be verified by a club official. To have your identification verified, please bring your application form and identification to club on a Wednesday evening once training has commenced and see Mel Wright (Secretary) for sign off.

Once your identification has been verified you can then forward your application to:

STEP 2: REGISTER BLUE CARD NUMBER WITH NSJWHC

 Once you have received your Blue Card, you will need to update your personal profile with your Blue Card number and expiry date. 

To do this:

• Log into your nsjwch.org.au profile page
• Click on name in the top right hand corner of page
• From drop down menu click on "Change my details"
• Enter Blue Card number and expiry date in fields provided
